You should also include the postal address to which you would like the sample delivered.

It may be useful to include a contact phone number or email address in case we need to clarify any of the fabric details you have provided.

This message will be passed on to our fabric supplier in order that they can send you the samples directly, but rest assured that the details you provide on this form will be used only for providing you the fabric samples you desire and for no other purpose.
